To Make an Appointment

Have your doctor’s prescription available. If you haven’t seen your doctor yet, feel free to contact us as you might not need a referral for an initial evaluation, otherwise we can assist in obtaining a referral for further treatment. Have your insurance information ready when you call or email the name of your insurance company. Check our list of Insurance Partners to see if we have a contract with your insurance company.

What to Bring

Along with the forms you printed and filled out, please bring the following:

• Your insurance card - Driver’s license or other picture ID - Your medical doctor’s prescription if you have one

• For Motor Vehicle injuries, please bring your car insurance information and/or third party information (if applicable).

• If you have a police report, include that as well.

• Comfortable clothing and shoes. You may change here in our office if necessary

• Bring your calendar so that we can schedule future visits

• If you have a deductible or co-pay, please be prepared to make a payment on your first visit.

• We accept Visa and MasterCard, personal checks and cash

 

 

On your first visit, be prepared  to undergo a complete physical exam which may take 45-60 minutes.  

We will be performing a comprehensive evaluation to determine your needs.

You may receive treatment, self-care tips and home exercises on your first visit.

 

If you have not yet spoken with the front desk about your insurance benefits, please be sure to call us with your insurance information before your first visit. We verify your benefits as a courtesy to you so that you are aware of your financial responsibility. We look forward to seeing you soon!
